Mentor Guardian Consent

This Mentor Guardian Consent (the “Consent”) is a legal agreement between you and MentorBuddies, LLC, a Massachusetts limited liability company (“MentorBuddies”).

By clicking on the “AGREE” link, you are agreeing that you will be bound by and are becoming a party to this Consent. If you do not agree to all of the terms of this Consent, click the “I DO NOT AGREE” link.

1.  Guardian Consent.

     (a) Services Agreement. You have received this Consent because your child or ward identified in the email you received from MentorBuddies (the “Minor”) is under the age of 18. The Minor has completed a profile with MentorBuddies and desires to be featured on the Site and receive the Services. The Minor has agreed to a Services Agreement confirming the terms of the Services, a copy of which is here (the “Services Agreement”). As part of the registration process, the Minor has been informed that the consent of his or her parent or guardian is required as a condition to the Minor’s receipt of Services from MentorBuddies and delivery of Lessons to Students pursuant to the Services Agreement. The Minor has provided your email address in order to provide such consent.

     (b) Guardian Consent. By clicking on the “AGREE” button, you hereby provide such consent, acknowledging and agreeing to the terms of the Services Agreement, as may be amended or updated from time to time in accordance therewith. If at any time you wish to revoke this Consent, you may do so solely with respect to Services to be provided following the date of such revocation, by delivering written notice of such revocation to info@MentorBuddies.com.

2. Guardian Representations. You hereby represent and warrant to MentorBuddies that (i) you are the legal guardian of the Minor and are responsible for the care, custody and control of the Minor; (ii) no other person’s authorization is required to grant the rights herein, and you have the full authority to consent to the Minor’s receipt of Services pursuant to the Services Agreement; (iii) you have read the Services Agreement, and have had an opportunity to obtain independent legal advice in connection therewith; and (iv) to your knowledge, the representations made by the Minor in the Services Agreement are true and correct.

3. General. This Consent shall inure to the benefit of the parties’ respective successors and assigns. Except for the Services Agreement, this Consent encompasses the entire agreement of the parties with respect to its subject matter and there are no other agreements or understandings, either written or oral, with respect thereto. This Consent may not be modified or amended except in a writing executed by the parties. This Consent will be governed by and construed in accordance with the laws of the Commonwealth of Massachusetts, without reference to its choice of law principles. With respect to any suit, action or other proceeding arising out of this Consent, or any other transaction contemplated thereby, the parties agree to exclusive personal jurisdiction and venue of the federal and state courts located in the Commonwealth of Massachusetts. All capitalized terms used herein but not defined shall have the meanings set forth in the Services Agreement.
